In the form you can respond to some or all of the following questions:

- What do you and your arts community need to be amazing - or even more amazing - in the future?
- What could Creative New Zealand do to work with you and your arts community to grow and thrive in the future?
- How can Creative New Zealand support arts and artists more fairly?
- What are some of the most important issues Creative New Zealand should put energy into in the long term?
- Is there anything else that you would like us to know?
